Has anyone had an insurance (champva) who does not required preauthorizathion ?

I have champva and they do not required preauthorization. They said that the only requirement they have is to have a bmi of 40+. They will do not send out approval letters since the do not preauthorize. My surg. office is having a problem with this. They can call and talk to my insurance directly but require it in writing. Anyone else had this problem?
