Pacificare HMO-anyone been approved/denied for post WLS abdominoplasty/skin removal?

I'd like to know what other people's experience has been with getting approval for abdominoplasty, skin removal surgery through Pacificare HMO--what documenation they required, results, was there an appeals process that you had to go through?
