Medicare - What's the process to have an abdominoplasty and hernia repaired covered by Medicare?

I need to make an appointment for a consult with a plastic surgeon regarding a ventral hernia repair and abdominoplasty. Will Medicare cover the abdominoplasty too and do I need pre approval. I also have chronic back pain which I have been treated for numerous times. I would like some info before I see the surgeon. Thank you.
