Has anyone been approved for WLS by State Farm?

I would like to know if anyone has been approved for surgery by State Farm Insurance. How long did you have the ins. before you requested approval, was the ins. purchased specifically for this surgery, were you denied at first or had no problems and what did they require from you before they approved?
