Does anyone know a good pcp in Union City/Hayward

I am looking for a pcp in Union City/Hayward willing to refer someone with a bmi of 47 (5'7" 300lbs) that is otherwise healthy (I do have a history of weight related knee dislocations) . I have small children and dont want to leave Kaiser but my current pcp will not refer me.
