Anyone with BCBC PPO from Illinois???????????????????????????????

I am looking for criteria for them for WLS. I work for United Airlines and am looking for someone who can give me an idea of what I need to get approved. I have a BMI of 55.7 and I have GERD, as well as back problems and I believe I have sleep apnea but has not been diagnosed by doctor. Any help would be greatly appreciated
