What pictures to they take for insurance approval?

I will be going to the doctor in about a month to start on the TT process. What pictures to they take to send to the ins company? Am I completely nude? Do I stay in my underwear? I think I will be in ALL my glory. My wife says they shouldn't be allowed to do that. So what's the answer?
