what insurance has in the past approved plastic?

looking to change hmo in aug. had wls in nov of 01 now down closre to 100 lbs go to the gym 3-4 times a week but still a lot of extra skin i need my tummy,arm's and what use to be my breast done they look like elephant knees. als o do not have any problem with my folds the only thing is i can not fit shirts in the arm and have to buy xl not l or m and i keep falling help with some hmo etc and info
