Documentation of diets

When you first register on this website it asks you what kinds of diets you been on if I am not mistaking mine automatically came up saying I have used Phen Phen and weight watchers. Well I need these documents but the doctor I went to for the diet pills has since retired and I did not transfer my records and this was back in 1994 and 1995 . Also I dont have proof that I was on weight watchers either because this was also a few years back too. I now this proof for Cigna for a approval could anyone tell me if there is records of this some where that I could get I am assuming there is how else would that have came up when I register. Please any help would be great Thanks Donna
