Help I need documentation of diets.

My surgeon said Cigna hmo requires documents proving you were on diets for at least 12 weeks I was in weight watchers but it was so long ago does anyone have general documents for this program that will be good enough for me to send to Cigna it has to track my weight in a 12 week period I can fill it out if anyone has a form. Any help is greatly needed. Thanks
