BCBS TRS Active Care - Appeal for Revision to RNY

I have been denied by BCBS TRS for a revision from VBG to RNY. They are saying that I need 12 mos of Dr. supervised diet. After my VBG in 2002 I was cut loose by my surgeon and my PCP was not that familiar with WLS. I logged in everything I ate, drank, exercise, etc. on fitday.com and was a member of Curves and worked out 3-4 times per week. I did this for 7 months until I began having problems with my VBG which ulitmately caused failure. I need help with the appeal letter/process.
