I don't have a pcp I have a nurse prac. who won't give referal

I don't have a pcp. I go to see a nurse practistioner. She will not give me a refferal to a dr because she says I just need to try more diet and exercise. I am currently 302pds I am 22 years old and have 2 children. I have been overweight all my life. I had my gallbadder removed a few years ago. Do I have to have a refferal by a pcp ? will a dr do the surgery without the refferal ? I has masshealth as insurance.
