Did anyone elses Ins. state that it did not cover Morbid Obesity surgically?

My insurance states that it does cover Morbid Obesity, but not surgically. Although, they did say to get all of the paperwork together and send it in for pre-determination. I am very confused, do you think that I stand a chance in being covered? Has anyone elses insurance ever stated this?
