Has anyone tried to get copy or their Psych Evaluation,and been refused???

Hi,Im wanting to hear form anyone that has tried to get a copy of their Psych Evaluation and been refused!!I paid 400.00 for the evaluation,went to the person the surgeon had arrangments with.I have filled out the paper work 4 times and requested the report.Both offices refuse to give me a copy:(Also I have requested they send it to my PCP.They have not done this either.It has been 3 months.I have gotten nothing but the run around from both offices.From what I have found out on the net,I have the leagal right to the records unless there is something harmful for me to see.(If there is they have not told me so they are not to conserned)If anyone out there knows Oregon Law and rights to psych eval report,and can tell me what to do next i would welcome the help.I have filled out the Medical Request 4x!(I will be having WLS with a diffrent surgeon)Im planning to write a letter send another request to each office and send them certified mail.I love this site,thanx in advance for all the help you all give!
