Does anyone have advice about appealing an insurance descision

I have had 2 verbal "yes your surgery is covered"answers and then. They sent me a written denial letter. I already have a date (apr 24th) and now what!I don't know where to go from here.Do I need to get an attorney? Can I appeal this myself.what are my chances of a reversal? Thank you!!
