Insurance co. refused to pay for consultation with surgeon, now what??

I just received an eob (explanation of benefits) from my insurance company (BC/BS Personal Choice PPO), that they are refusing to pay for the consultation with the surgeon, because they don't cover for that diagnosis. I don't know what diagnosis the surgeon's office listed. Has this happened to anyone, and the surgery was still approved? I am less than 1 month, before my surgery date. I can't believe this is happening!! I also have my pre-op testing this week. Thanks for any info.
