Has anyone been approved through UHC in Kansas or Missouri?

I am looking to correspond with anyone in Kansas or Missouri that has been approved for WLS and who has United Healthcare Choice Plus Select POS. If I get denied I want to provide evidence to the appeal board that they have in fact covered this surgery in the past and who have the exact same policy as me. My policy is UHC of the Midwest.
