Doctor not following through

Has anyone had a problem with their doctor not following through when your Insurance denies you the first time? My doctors office has all the paperwork my Insurance requested and they still havent faxed it to my Insurance and trying to get a call back from the Dr. is virtually impossible Any Suggestions?
