Is my insurance crazy?

My insurance has alredy approved my sugery. They done this last month. My sugery in June 28. My question is i just got the statement from them where i went to the Doctor for the first visit and they will not pay. The form said the reason is because they will not pay for weight reduction doctors. I am so comfused what does this mean? How could they approve the operation and not the Doctor visit?
