Anyone out there know Dr. Jaecks in Salem, Or.?

I have Kaiser and I have an appointment with Dr. Jaecks in Salem, Or on 5/25/00 about my surgery but can find nothing our about him. I even e-mailed Kaiser and they told me to talk to Member Ship Services. I'm still waiting to hear from them.
