How do i get started I dont have insurance or a Dr

I dont make alot of money and i have 3 kids to suport.Can you get the surgury without a pcp.Will they help me figure out how to pay for it? the only time i went to a dr is when i was pregnent.I dont know for sure what being heavy has done to me for sure yet.
