Which insurance is most likely to approve surgery?

I have a choice of insurance carriers. Does anyone have any recommendations as to which insurance carrier is more likely to approve bypass surgury with the least amount of contest? I'm in Colorado but would appreciate advice from any state. Thanks ahead of time!
