Is there a way to fight for approval even if my insurance excludes this procedure?

I haven't asked for approval yet. But here's what my insurance manual says. "Health Services and associated expenses for the surgical treatment and non-surgical medical treatment of obesity (including morbid obesity) are excluded." Is there a I way I can prepare for a fight if and when I'm denied?
