anyone know the newest stipulations for cigna ppo?

im just wondering what new stipulations cigna has. im going to try to get the surgery,but i want to be well armed when it comes to the insurance company. someone told me that they just recently said that now you have to have been on a doctors diet in the last year for at least 6 months. anyone know anything else that could help in the insurance proccess???? thanks
