Do we need copies from the places we attended like Nutri System?

I have attended Weight Watchers, Nutri System, Quick Weightloss Center, Bally's etc. Do I just list them with the time frame they were in and the weight I lost? Or do I need to track them all down and get a copy from them saying I was there on letterhead stationary?
