Does not being on supervised physican diet hurt chances of surgery?

Hve been over 200 pounds for atleast 6 years and over 240 for about a year. Have not really been on supervised diet due to one bad experience. Hve tried the over the counter pill with diet and exercise, Will this be a cause for non approval for surgery?
