Has anyone gotten approval from US Healthcare HMO in NY since 1/1/03?

Is it true that now the ins. co. wants documentation? Is it not enough for my PCP to write dates, programs, etc. for the past 6 months? Do I have to show I've seen a dietician?I'm almost done with all my pre-op visits and am getting worried now that I won't be approved. If anyone has been approved by them please let me know. Thanks
