How do you talk to your PCP and tell him his letter is not good enough

I am almost all done with all my pre-op testing and I got a call yesterday from my surgeons office that my Primary's letter is not good enough, not enough details in the letter, it was 1 paragraph and that took him 2 weeks to write. How do I ask him to write specific weight loss programs and dates ?For the last 9 years he has always told me I need to lose weight, usualy when I'm in there for a cold or ear infection etc., I then try weight watchers or some diet, see him again in 6-9 months for another minor illness and he reminds me again. This has gone on forever. I've lost, I've gained back and forth but I realy don't think anything was ever documented. Do I tell him what programs I've tried and ask him to write it? But legaly can he, if it's not documented in my file, he will have nothing to back himself up?
