How do I seek a revision?

Am I doing this right. I made an appt with gastroenterologist but it is not till October. SHould i contact insurance company first about revision or do it this way and then ask. My insurance is choice care select and it does have an exclusion for all weight loss surgeries excepy VBG. Found this in the book. I had an RNY in September 99. Not sure which way to go and need help
