Is there a WLS friendly doctor with Humana Choice care select

Actually this is two questions. I am seeking a revision and am very afraid I will be turned dowm. I have Humana Choice Care Select. I am looking for a new PCP and a WLS Bariatric surgeon on my HMO Any Buckeyes that have this can you help. I have put back almost all of my weight. I have major health issues but no high blood pressure 150/80. Peripheral neuropathy no diabetes depression-Who wouldn't be
