Any one have Blue Cross of California Share 2500 Insurance?

Blue cross finally approved the surgeons and cut a check to them but they will not pay for the hospital or anetheisiologist because they billed the primary diagnosis as morbid obesity. The hospital (St. Agnes-Fresno) won't change the diagnosis, which they were given the codes by the dr. prior to surgery. Any one know what to do??
