Need advice on individual BC of ID plans!

I have the application for Blue Cross of Idaho. If anyone has any experience on which plan is best for covering WLS PLEEEASE contact me. I only have until Tuesday to get it FAXED in for coverage for March before the rates go up. Has anyone used "Personal Blue" Major Medical 1000, 2000, 3000, or 5000????
