My insurance states limited in or excluded from coverage

I have HealthNet it states services and/or procedures are either limited in coverage or excluded from coverage under this health plan. The policy listed just (Obesity) alone, not morbid or extreme. Can anyone help? Also ,I have a recurrent tear in my left knee. I went to see Orthopaedic surgeon, he states he would like to do the surgery, but does not recommended doing orthopaedic surgery on my knee. He goes on to mention why, due to being extremely overweight (305). He did put that in a letter. Also due to other health related problems my PCP gave me a medical necessarty letter as well. Has anyone heard of having any problems with being deny for the gastric bypass
