Anyone in Florida have Capital Health Plan?

Has anyone living in Florida that has Capital Health Plan been approved for WLS or have any suggestions about getting approval with Capital Health Plan. I know that before I can get a referral to see a surgeon I have to see a dietician for 6 months, who I can't even get in contact with.
