Any ideas on how to persuade Dr. to give a referral?

12/19-I need help in persuading my pcp to give me a referral for a consultation. He believes that it is "cosmetic" and not really necessaary. I've faxed him info from the policys of Aetna. no response. My current weight is 284 lbs. -bmi 42 Can someone help?
