Has anyone tried getting approval from Mutual of Omaha recently?

I recently started a new insurance - Mutual of Omaha, and my paperwork was submitted for pre-authorization. My ins. co. called the surgeon last week and said it was an exclusion, but I know its covered if "medically necessary". So now I move on to the appeal process. Anyone have advice to share regarding this insurance company? Thanks.
