I need info. or a copy of a letter to send along with my packet for insurance approva

I have noticed on this sight that my carrier for insurance has an 50/50 approval rate and would like to send a letter of plea along with my packet for the surgeons office to send to my Ins. Besides my diet history and health issues I'm at a lose for words. Any advice will help,My carrier is B/C,B/S Alliance of MO. Thank you!
