Does anyone have an appeal letter they sent for themselves?

I would like to read any letters you have sent that may be able to help me. I was denied with a Dr. that was "out of network." So I have another appt. with an "in-network dr." My RN Case Manager said that she would submit this new pre-cert as a new one instead of an appeal since I'm going to another Dr. I also found out what specifically they are wanting and am getting these together. I feel though that a letter from myself would help. I have tried to write one myself but I just want to see others. Thanks Brenda Kay Brenda Kay
