Preapproval letter...who sends?....

Mine and my husb ins co will deny my WLS coverage due exclusion on both...but I'm going to fight it and go through Lindstrom...But....Who does the letter?...I don't have a problem doing it, My PCP doctor said she would sign anything and the surgeon I'm thinking about doesn't want to bother cause of the exclusions...Should I just do it and have my PCP Sign? Do I have to say it's that particular Surgeon? Thanx
