HELP! I need help with Insurance Appeal

I am on my third and final Formal Appeal to TRICARE Management Activity (TMA) for a Breast lift/reduction with implants, abdominoplasty, thighplasty, brachioplasty, & hip-plasty. I do not know what else to give them. Please if you have any further information please let me know???? I'm lost.
