Has anyone been approved, then disapproved due to the employer's exclusions?

I was approved over the phone and then in a letter from Tufts. My doctor's office called to tell me it was a mistake (I confirmed this). It seems that my husband's company pays out for surgeries and they exclude this, not Tufts. Tufts did tell me to file an appeal, but gave me little hope. I can't stop crying.
