Any recommendations for a letter after a denial?

BC/BS has denied me say I don't meet the critea. My bmi has be over 40 since 1997, (no documentation before that) I am 150lbs overweight, I have had my phyc. evaluation and my Dr. has said it is medically necessary for me to have this surgery to lose weight. I have comorbidities. I need help. What more do they want? I asked them and then told me I didn't fit the guidelines. I gave it to them in Black and White. Any suggestions? PLEASE.
