Has anyone received a denial from this company?

I am a teacher and I am currently at 384 lbs with diabetis and reflux and other conditions and have verbally heard from the insurance company that I will be denied. I talked to the insurance person at my district and they said that they did not know of any company dealing with BC/BS that ok the surgery and that if I went to appeal that the doctor would have to certified that I am in imminent danger of death to be listened to and even then there would probably be a denial. I really want this surgery and I need to get some advice. Can anyone help. Liz
