I was told by my surgeon that cigna wants documentation of diets

I was told that cigna wants documentation of medically supervised diets or programs.((3)well-documented diet programs within the last 5 years,1 of which needs to be in the last 12 months, and each diet must be 12 weeks long). I have been on diets for the last 10 years, but have not been able to afford to see doctors for diets, and do not have 3 medically supervised diets with documentation. Does anyone know what else I can do? Has anyone been approved with out the above documentation. Any advice or help would greatly be appreciated. I am desperate to get surgery. HAPPY 4 JULY!!!
