Has anyone had Physician's Plus Ins (Madison Wi) approve their surgery?

If anyone has info or can help me get pre approval-let me know. I meet all requirements for surgery, my MD is referring me to a surgeon, but this is not a covered procedure in my ins plan (HMO)
