I was approved to have surgery by dr. Keith Marshall, who is not allowed to do it now

Now I am in a catch 22..... After fighting with my insurance company since Feb 21st (my scheduled surgery date), they finally gave me premission to see Dr. Michael Hocking, only to find out that he is not taking new patients till after the new year. My insurance says I must stay in Florida. I can't find a doctor now, after all the fighting I did with the insurance company, it just does not seem fair. I have a supervisor at the insurance company trying to help me now atleast, but if you have any suggestions, they would be greatly appreciated. Also, does anyone know whats going on with Dr. Marshall now?
