Does anyone know the criteria that Univera uses in order to approve for WLS?

I am looking into WLS and I am concerned about my insurance company. I have looked through most of the info on this site and I find no mention of my insurance company. I just would like to know the criteria that they go by so I know what my next move will be. If anyone can be of any help with this I would greatly appreciate it.
