A site for your letter help....

Go to http://www.clos.net That is a site for a doctor in NC. His site is a great site with TONS of great WLS information AND it has a list of different insurance letters that were written. On the main page, page down, looking down the left side and you can find them under INSURANCE, I believe. GOOD LUCK!
