Does anyone know anything about BC/BS of AZ, PPO?

I am changing my insurance in June to BC/BS of AZ, PPO. Did this insurance give anyone any problems with approving weight loss surgery? I have an HMO right now, and they shot me down. Thus, the change to the PPO. Does this insurance also demand specific pre-ops....like a psych eval, sleep study, etc.? Any and all info would be extremely helpful!
