No serious co-morbites other than severe depression

I had a complete physical this week and my doctor found no serious medical problems other than severe depression. My BMI is 51% and I am beginning to have arthritis and frequent back aches. Are these conditions enough to consider the surgery medically necessary? My ins. is Aetna POS in VA.
